When I dealt with insurance client's, I actually had the client:
·  Pay me up front and didn't charge them extra
·  Made a copy of the SOAP Notes and sent it to their insurance company
·  Had the client sign a form stating that they were responsible to get reimbursed from the insurance company and that I was not r esponsible if they didn't receive full payment
·  Gave the client receipt's for each treatment
·  Write a scenario of what went on in all the treatments
