When I dealt with insurance client's, I actually had the client:

• Pay me up front and didn't charge them extra
• Made a copy of the SOAP Notes and sent it to their insurance company
• Had the client sign a form stating that they were responsible to get reimbursed from the insurance company and that I was not r esponsible if they didn't receive full payment
• Gave the client receipt's for each treatment
• Wrote a scenario of what went on in all the treatments
  
  As you can see, it's still a lot of work; but you are guaranteed the money up front.
